What do you need to do?

Make a new Texas Medicaid application

Click here for: Online Enrollment

  • First-time applicants
  • Re-applicants
  • Existing providers with any of the following changes:
    • New Medicare number
    • Change of ownership
    • Change of provider status (individual, group, performing provider, or facility), for example, a performing provider leaving a group practice
    • Change or addition of a provider type
    • Opening of additional practice locations

Apply to become a Texas Health Steps (THSteps) provider

Apply to become a dental provider. Dental providers must complete a separate enrollment application for each practice location.

Note: If enrolling as a Doctor of Dentistry practicing as a Limited Physician, providers must complete the Medicaid Provider Enrollment Application Form.

Apply to become a Children with Special Health Care Needs (CSHCN) Services Program provider.

Pharmacy providers are eligible to enroll in THSteps Medicaid Children’s Services (Comprehensive Care Program (CCP)). To be enrolled in Medicaid Children’s Services, the pharmacy must first be enrolled in the Medicaid Vendor Drug Program (VDP).

The THSteps Medicaid Children’s Services program may cover some over-the-counter drugs and disposable or expendable medical supplies for clients birth through 20 years of age available in pharmacies that are not covered by VDP. Pharmacy providers must continue to bill HHSC-VDP for drugs covered under VDP.

Apply to become a provider for Long Term Care (LTC) programs

TMHP's Provider Enrollment Unit does not enroll providers for LTC programs. LTC program providers should contact the Texas Department of Aging and Disability Services (DADS) on the DADS website at www.dads.state.tx.us/business/index.html.

95-Day Filing Deadline and 365-Day Federal Filing Deadline
Information posted October 1, 2008: Reminder: Providers are required to meet the 95-day filing deadline and additionally the 365-federal filing deadline. As stated in the 2008 Texas Medicaid Provider Procedures Manual, section 5.1.4 "Claims Filing Deadlines," claims submitted by newly enrolled providers must be received within 95 days of the date the new provider identifier is issued and within 365 days of the date of service (DOS). In accordance with federal regulations, all claims must be initially filed with TMHP within 365 days of the DOS.
