If you are a new PCCM client, you need to choose a primary care provider. Look carefully at the letter you received in a welcome packet sent to you in the mail. The PCCM Handbook and PCCM Primary Care Provider and Hospital List are also in the packet. The letter tells you how much time you have to choose a primary care provider. If you do not choose a primary care provider, you will be assigned one.

You can search for a new doctor on the TMHP Online Provider Look-up . Call us toll-free at 1-888-302-6688 (TDD 1-800-735-2988) to choose your new doctor. After you choose a new doctor, we will send you a letter telling you the change was made. Keep the letter. Take the letter and your current Medicaid ID form to every doctor’s visit.
