Medicaid services for you or your children will be provided through PCCM. You can get:

  • All regular Texas Medicaid services.
  • A doctor who helps you manage your health care.
  • Medicine from drugstores that take Medicaid.
  • Free home delivery of medicine from some Medicaid drugstores (ask your drugstore if they offer home delivery).
  • Texas Health Steps  medical and dental check ups and care from birth through 20 years of age.
  • Free transportation or money for gas through the Medical Transportation Program.
  • Reminders about your children's check ups and shots.
  • Referrals to specialty doctors when you need them.
  • A nurse to help you with your health care. A nurse is ready to help you 24 hours a day, 7 days a week. To find out more, dial 1-800-304-5468. The call is free.
  • Answers to your questions about your Medicaid benefits. Dial 1-888-302-6688.
  • Help if you are pregnant or think you may be pregnant. Dial 1-888-276-0702.
  • Help from Community Health Services. This service helps you and your family to be healthy. To find out more dial 1-888-276-0702.


There are some services that you can get without going to your primary care provider. These services are:

  • Family planning services and supplies such as birth control.
  • Medical and dental check ups, treatment, and emergency dental care for children through Texas Health Steps.
  • Mental health and substance abuse treatment.
  • OB/GYN care from a Medicaid OB/GYN, family practice, or internal medicine doctor.
  • Routine eye care for glasses.
  • Eye care services from ophthalmologists or therapeutic optometrists for eye care services (but not surgery).
  • Immunizations for children.
  • 24-hour care in an emergency room.

If you are a new PCCM client, you need to choose a primary care provider. Look carefully at the letter you received in a welcome packet sent to you in the mail. It tells you how much time you have to choose a primary care provider. If you do not choose a primary care provider, you will be assigned one.

Yes, you can change your primary care provider. You can search for a new doctor on the TMHP Online Provider Look-up. Call us toll-free at 1-888-302-6688 (TDD 1-800-735-2988) to choose your new doctor. After you choose a new doctor, we will send you a letter telling you the change was made. Keep the letter. Take the letter and your current Medicaid ID form to every doctor’s visit. Call the PCCM Client Helpline at 1-888-302-6688 (TDD 1-800-735-2988).

 Community Health Services (CHS) 1-888-276-0702

PCCM includes free Community Health Services. Community health coordinators who are health educators, nurses, diet specialists, and social workers will work with you and your primary care provider to help you get the most from the PCCM.

You can get help with health care issues like: 

  • Learning about an illness 
  • Preventing illnesses from getting worse 
  • Tips on parenting 
  • Healthy eating and weight loss
  • Taking care of yourself if you are pregnant
  • Keeping your children from getting sick 
  • How to stop smoking 
  • Finding medical supplies 
  • Getting the most out of your medical visits 
  • Understanding your Medicaid benefits

PCCM can also help you find different programs in your area. These programs may include childcare, rides to doctor visits, therapy programs and much more.

PCCM Nurse Helpline 1-800-304-5468

If you or a family member is sick, you can call the nurse helpline to talk to a nurse. The nurse helpline is free to everyone in PCCM. You can call the nurse helpline at 1-800-304-5468, 24 hours a day, 7 days a week.
The nurse helpline can answer questions about you or your child's health in English, Spanish, or another language. A nurse can give you medical information to help you decide if you:

  • Can take care of yourself or your child safely at home.
  • Should talk to your doctor.
  • Should go to the emergency room.

The phone call is free. Please have your Medicaid ID number with you when you call. Your Medicaid ID number is on the Medicaid ID form you get each month in the mail.
