29.2.23.7 DTaP-IPV-Hib Vaccine (Pentacel)
DTaP-IPV-Hib contains 3 state-defined components. Procedure code 90698 must be submitted in addition to the most appropriate vaccine administration procedure code and modifier U3.
Procedure code 90698 is processed as informational; no reimbursement is made for the cost of the vaccine. The administration fee may be reimbursed. CSHCN Services Program providers may obtain the DTaP-IPV-Hib from the TVFC Program for clients birth through 18 years of age.
