24.2.10 Urinalysis

Urinalysis must be billed using the most comprehensive code that describes the urinalysis test rendered on that day. If individual components of a urinalysis are performed on the same day, they may not be billed separately. If two or more of the components are performed on the same day, they must be combined and billed with the urinalysis procedure code that most accurately reflects the test provided.

If procedure code 81015 is billed in addition to procedure codes 81000 or 81001 on the same day by the same provider, procedure code 81015 will be denied as part of procedure code 81000 or 81001.

If procedure code 81015 is billed with urinalysis procedure code 81002 or 81003 on the same day by the same provider, both procedure codes are denied, requiring paneling into either procedure code 81000 or 81001.

If procedure code 81015 is billed separately with one or more of the urinalysis components (procedure codes 81000, 81002, 81003, and 81005) on the same day by the same provider, both procedure code 81015 and the other component are denied, and should be combined and billed as a urinalysis.

If procedure codes 84578 or 84583 are billed on the same day by the same provider as procedure codes 81000, 81001, 81002, 81003, 81005, or 81020, procedure codes 84578 or 84583 will deny as part of another procedure.
