26.2.10 Neurostimulator Supplies
Neurostimulator supplies, including leads and electrodes (procedure code A4290 or L8680), are a benefit without prior authorization only if there is documentation of a client-owned device. Additional documentation, such as the purchase date, serial number, and purchasing entity of the device may be required.
