5.6.1 UB-04 CMS-1450 Electronic Billing

Electronic billers must submit UB-04 CMS-1450 claims with TexMedConnect or approved vendor software that uses the ANSI ASC X12 837I 4010A format. Specifications are available to providers developing in-house systems and software developers and vendors. Because each software package is different, field locations may vary.
