8.6.14 PCCM-Enrolled FQHCs and RHCs Must Report Medical Staff Changes to TMHP

FQHCs and RHCs that are enrolled in PCCM must inform TMHP of all changes to their medical staff so that the PCCM Provider Directory can be properly updated. Existing PCCM enrolled FQHC and RHC facilities can view their current list of associated providers on the Online Provider Information Change (PIC) form on the OPL, and make changes.

An accurate and up-to-date provider listing gives PCCM clients access to all of the staff members of these providers. FQHCs and RHCs that do not update their provider listing online must complete the Form 8.6, "Primary Care Case Management (PCCM) FQHC and RHC Medical Staff Update Form" of this section. The form can be faxed to the PCCM Contract and Credentialing Department at 1-888-235-8399.
