O.4.2.1 Annual Family Planning Examinations
The annual family planning examination (procedure code 99204 or 99214, modifier FP, and diagnosis code V2509) consists of a comprehensive health history and physical examination that includes medical laboratory evaluations as indicated, an assessment of the client's problems and needs, and the implementation of an appropriate contraceptive management plan. For the annual family planning examination, only diagnosis code V2509 can be used. The annual examination is allowed once per fiscal year, per client, per provider. Subsequent visits within the same year must be billed as an office visit or other outpatient visit.
