1.1.3.2 HHSC Medicaid Provider Agreement
The HHSC Medicaid Provider Agreement must be submitted by all providers who enroll in the Texas Medicaid Program and must be signed by the person applying for enrollment. If the applicant is an entity, a Principal of the entity must sign the application. "Principal" is defined in "Provider and Principal Information Forms" . This form is an agreement between HHSC and the provider performing services under the State Plan wherein the provider agrees to certain contract provisions as a condition of participation. All pages of the agreement must be present with the enrollment application even if the forms are blank because they are not pertinent to the provider's situation.
