19.2.1 Categories of Practice

All dental providers must declare one of the following categories:

•General practice.

•Pediatric dentist.

•Periodontist.

•Endodontist.

•Oral and maxillofacial surgeon.

•Orthodontist.

•Other (prosthodontist, public health, and others).
