3.1.8.1 Requirements for Stem Cell Transplant Center Enrollment
A stem cell transplant center must be compliant with all of the criteria specified in the most current "Guidelines for Clinical Centers" on the American Society for Blood and Marrow Transplantation (ASBMT) website at www.asbmt.org/policystat/policy_op.html. Additionally, the Required Information for Stem Cell Transplant Facility Providers Only form, provided with the CSHCN Services Program Provider Enrollment Application, must be completed and submitted with the application.
