3.1.8.2 Requirements for Cleft/Craniofacial (C/C) Center Team Enrollment
To be enrolled by the CSHCN Services Program, C/C center teams must comply with the following requirements:
• C/C center teams must include at least the following participants: a plastic surgeon and/or oral surgeon; an otolaryngologist; a primary care physician; an orthodontist and/or a pediatric dentist; a master's level licensed speech language pathologist; a client educator; and a designated C/C center team care coordinator who also may be one of the previously listed participants.
• C/C center teams shall ensure that each client has a C/C center team care coordinator to assure that the focus of the service is client and family-oriented, and that the client/family and the C/C center team jointly develop a comprehensive treatment plan.
• Each C/C center team must identify an administrator who is responsible for coordinating and maintaining C/C center team records and assuring that the C/C center team adheres to CSHCN Services Program rules and regulations.
• C/C center teams must complete the required information and attestation in the CSHCN Services Program provider application.
• Contact information, including address, telephone number, and email address, must be included for the team administrator.
• C/C teams must update any changes with their team membership in a timely manner, or at least every two years with the CSHCN Services Program claims contractor, TMHP.
To facilitate statewide coverage, the CSHCN Services Program also may enroll affiliated providers. CSHCN Services Program-covered affiliated providers must be linked with a CSHCN Services Program-enrolled comprehensive team and must ensure coordination of client management as needed. At the time of application, affiliated providers must specify the comprehensive C/C center team with which they are linked or affiliated.
