24.1.2 Group Practices
A provider group participating in the Texas Medicaid Program must complete the CSHCN Services Program provider enrollment application, available through TMHP, to enroll in the CSHCN Services Program. The CSHCN Services Program application must include the Medicaid group provider identifier and performing provider identifiers for each physician within the group.
If additions or changes occur in the group's enrollment information after the enrollment process is completed, the CSHCN Services Program provider group must notify TMHP of the changes.
