24.3.9.1 Authorization Requirements

All implants and associated aural rehabilitation must be prior authorized. The following information must accompany the request for prior authorization:

Documentation from the audiologist and otolaryngologist indicating that the client is a good candidate for the procedure and meets the requirements outlined earlier in this chapter.

Documentation that a referral to an appropriate aural rehabilitation provider is in place.

Documentation from the client's primary physician, neurologist, or school diagnostician that the client has the cognitive ability to use the implant.

Replacement of rechargeable AA batteries must be prior authorized.
