24.3.9.2 Sound Processor Replacement Guidelines
Unless ordered by a physician, the processor must be used for 12 months before replacement of the unit is considered for reimbursement. Replacement of a sound processor requires prior authorization. The physician must submit documentation of medical necessity when requesting prior authorization for the replacement of the sound processor.
