Section 1 - Select search criteria below to find a provider that is accepting new patients:
Health Plan: Select the type of Medicaid in which you are enrolled. If you are unsure, look at your Medicaid ID card (Form 3087) to find out. Here are descriptions of the different Medicaid types:
Traditional Medicaid - This is also called fee-for-service Medicaid. In this program, you can go to any doctor you choose as long as they take Medicaid.
Primary Care Case Management (PCCM) - In PCCM, you have a primary care provider for your health care. A primary care provider is a doctor, clinic, health center, physician assistant, or specially trained nurse. Your primary care provider takes care of you most of the time but sends you to a specialist when you need one.
STAR—In this program, you select a health plan and a primary care provider.
STAR+PLUS—In this program, you select a health plan and a primary care provider.
If you do not know the Medicaid program in which you are enrolled, please call the TMHP Contact center at 1-800-335-8957.
Section 2 - Enter other information to narrow the search:
Last Name/Facility Name: If you want to search for a specific provider, enter the provider's last name or the name of the facility, hospital, medical group, or clinic.
HMO Plan Name: Enter the name of your health plan to search for a doctor or other provider that is part of that health plan. If you do not know your health plan's name, you can:
Look at your Medicaid ID card (Form 3087).
Call the TMHP Contact Center at 1-800-335-8957.
Click Any to search all health plans.
Service Area: A group of counties in which a managed care plan, such as a health maintenance organization (HMO) or the Primary Care Case Management (PCCM) Program, provides Medicaid services.
Provider Type: If you want to search for a type of doctor or other provider, choose the provider type from this list:
Primary Care Provider - Provides most of your health care.
Specialist - Treats only certain illnesses or parts of the body.
Hospital/Facility - Hospitals and other health-care or treatment facilities, such as clinics.
Texas Health Steps Medical - Provides health-care services for those 20 years of age and younger. These services include check-ups, immunizations (shots), vision and hearing screenings, and health education.
Texas Health Steps Dental - Provides dental health-care services for those 20 years of age and younger. These services include dental check-ups, treatment, and emergency dental care.
Mental Health Services - Hospitals, facilities, and individual providers that perform mental health services.
Vision-Specialists that provide eye and vision care.
Section 3 - Enter location information:
Zip Code: Enter the 5-digit ZIP Code of the area that you want to search. If you want a doctor or other provider close to home, enter the ZIP Code in which you live.
Distance From Zip Code: Choose how far away from a ZIP Code you would like to search. You will only be able to choose the distance after you enter a ZIP Code.
Provider Located in County: If you want to search a county, choose the county name. This will usually be the county in which you live. You will not be able to choose a county if you made an entry in the ZIP Code field.
Section 4 - Select other information to narrow the search further:
Accepting New Patients: Choose one of the following:
Accepting New Patients - Choose this if you want to search for doctors and other providers that are accepting new and current patients with Medicaid coverage.
Current Patients Only - Choose this if you want to search for doctors and other providers that are only accepting patients they have seen before.
Not Accepting Patients - Choose this if you want to search for doctors and other providers that are not accepting patients with Medicaid coverage at this time.
Provider Specialty: If you want a doctor or provider that treats only certain illnesses or parts of the body, choose a specialty. For example, to find a provider that only treats children, choose Pediatrics from the drop-down menu. If you choose All Specialties, they will all be searched.
Provider SubSpecialty: If you need a doctor or provider that is more specialized, choose a subspecialty. For example, you can search for a provider that has done bone marrow transplants.
Extended Office Hours: If you want to find a provider that sees patients outside of normal business hours, choose Extended Hours. If you want a provider that sees patients on weekends, choose Weekend Hours. Normal business hours are weekdays from 8 a.m. to 5 p.m.
Medicaid Waiver Program: Choose this if you want to find a provider that offers Medicaid services within the medical assistance program. Currently, the only choice is the Women's Health Program.
Other Services Offered: This is a list of all of the special services the provider can help you with.
First Dental Home (Dental for Children Under 3): This service is for children who are 6 through 35 months of age. It gives kids their first dental care.
Hearing Services for Children – This is the evaluation and diagnosis services of hearing loss, hearing aids, and hearing aid accessories for children from birth through 20 years of age.
Oral Evaluation—Medical Home: Some Texas Health Steps (THSteps) primary care providers have this service. The primary care provider will look at your child's teeth at a THSteps check up. The primary care provider will give you names of dentists. You can choose one of these dentists to be the dental home for your child.
Other Languages Spoken: If you want a provider that speaks a language other than English, choose a language.
Patient Age: Some providers only treat certain age groups. Enter your age or the age of the person looking for a doctor to find providers that treat that age group.
Gender Served: Some doctor treat just males or just females. Enter your gender (male or female) or the gender of the person looking for a doctor.
County Served: If you want to find a doctor or provider that serves patients from a specific county, choose the county in which you live.

No Results Found Page If no results are found that match your search criteria, you will get the following message:
No results found. Please press the back button on your browser to change your search criteria. You may try the search again with fewer limitations.
Expanded Results:
Primary care provider check : The primary care provider symbol marks the health plans the provider is part of. If there is more than one health plan, the provider may be a primary care provider in some of them but not all of them.
