Attention: All LTC Providers

Information posted July 28, 2009: TMHP has identified a small number of instances (affecting relatively few providers) where multiple ‘child’ forms have been created and associated with a single ‘parent’ Document Locator Number (DLN). This has occurred with both Minimum Data Set (MDS) and Medical Necessity and Level of Care (MN and LOC) assessments as well as with Forms 3618 and 3619. This situation should not occur and is creating processing difficulties for the forms involved. To resolve this, TMHP will be removing certain pre-identified ‘child’ forms from the Long Term Care (LTC) Online Portal through a manual process. TMHP has contacted all affected providers via phone to alert them to the situation and elicit input to determine which forms should be removed from the LTC Online Portal.
