Change in PCCM Primary Care Provider Assignment Effective February 1, 2007
Information posted February 2, 2007: Effective February 1, 2007, the primary care provider selection process allows new Primary Care Case Management (PCCM) clients to choose their primary care provider before one is assigned. Previously, PCCM clients were assigned a primary care provider at the time of enrollment. Click on the title for more information.

In the new process, newborns have up to 105 days to select a primary care provider.  All other PCCM clients have up to 75 days to select a primary care provider. If clients do not make a selection within the specified time period, a primary care provider will be assigned.

 

If the Medicaid Identification Form (H3087/3087) lists either PCCNEWB01 (newborns) or PCCPCCM01 (all clients except newborns) as the primary care provider, any PCCM enrolled primary care provider can render health care services to the client.   

PCCM clients can continue to call the PCCM Client Helpline at 1-888-302-6688 or mail the Primary Care Provider Selection Form to TMHP to select a primary care provider.
