Prior Authorization Requests Received With Typed Font Signatures or Initials Require Date Stamp and Logo or Seal

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to eligible clients. Administrative procedures such as prior authorization, pre-certification, referrals, and claims/encounter data filing may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the client's specific MCO for details.

Texas Medicaid & Healthcare Partnership (TMHP) Prior Authorization (PA) will accept requests with signatures or initials generated through Adobe® Acrobat using typed fonts.

Typed font signatures or initials, such as available through Adobe Sign, will be accepted only with a date stamp and logo or seal. An example of typed font from Adobe Acrobat is shown below and will no longer be accepted:

Effective September 1, 2020, TMHP PA will return requests to providers if the request form has a typed font signature or initials without a date stamp and logo or seal.

All other electronic signatures or initials previously identified as acceptable that include date and time stamp and a logo or seal will continue to be accepted.

Note: Alterations must be made with a single line strikethrough and may not have the information covered by Wite-Out or any other obstruction to be considered.
