Reminder of Valid Electronic Signature Criteria

Last updated on

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to eligible clients. Administrative procedures such as prior authorization, precertification, referrals, and claims/encounter data filing may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the client's specific MCO for details.

The Texas Medicaid & Healthcare Partnership (TMHP) would like to remind providers to adhere to signature and date requirements for prior authorization and other required forms that are kept in the client record. Other required forms include plans of care, physician orders, evaluations, and letters of medical necessity.

The following are acceptable types of signatures:

  • Electronic or digital signatures that are created by software, which produces a digital system-generated date and time stamp or includes the logo or seal of the digital software that is used
  • A signature audit trail

TMHP will not accept the following types of signatures:

  • A photocopy or ink stamp of a handwritten (wet) signature
  • Typed signatures
  • Stamped images of handwritten (wet) signatures

For more information, call the TMHP Contact Center at 800-925-9126 or the TMHP-CSHCN Services Program Contact Center at 800-568-2413.
