Provider Enrollment on the Portal Instructions
Important: You must be enrolled in Traditional Medicaid before enrolling in CSHCN or Texas Health Steps. Select "Add to an Existing Enrollment," then click "Apply Filters" if you would like instructions for adding CSHCN or Texas Health Steps to your existing enrollment.

Texas Provider Identification (TPI) Table

Instructions about the TPI Table

You cannot make changes to the name, enrolled Programs, National Provider Identification number, Texas Provider Identification number, business structure, or Provider Type/Specialty. If you want to make changes to any of these fields, select Enrollment from the navigation bar at the left to and choose "New Enrollment" or "Existing Enrollment."

  • Select a TPI from the list.
  • If the TPI that you want to revalidate is not in the table below, please verify that the provider has an active enrollment and has not been disenrolled.
  • If you can't select the TPI, it is not eligible for the chosen enrollment application type. If you think that the TPI should be eligible for the enrollment application type, contact the TMHP Contact Center at 1-800-925-9126.
