Note: The Health and Human Services Commission (HHSC) has requested that TMHP publish the following information:

Note: This article applies only to claims submitted to TMHP for processing. Refer to the Medicaid managed care organizations (MCOs) for information about MCO benefits, limitations, prior authorization, reimbursement, and MCO specific claim processing procedures.

On April 1, 2016, the Centers for Disease Control and Prevention (CDC) announced the discontinuation of HPV4 (procedure code 90649) and HPV2 (procedure code 90650) from the Vaccines for Children Program (VFC). Therefore, these vaccines are not available for ordering through the Texas Vaccine for Children (TVFC) Program.

TVFC stopped distributing HPV4 and HPV2 as of March 31, 2016. Since the shelf life for HPV4 is 36 months, procedure code 90649 will no longer be a benefit of Texas Medicaid for dates of service on or after April 1, 2019. Since the shelf life for HPV2 is 48 months, procedure code 90650 will no longer be a benefit of Texas Medicaid for dates of service on or after April 1, 2020.

Providers may continue to submit a claim for vaccine administration when administering HPV4 or HPV2 (procedure code 90649 or 90650) while it is still available in the provider's stock.

In the absence of HPV4 and HPV2, HPV9 may be used to continue or complete the HPV vaccine series for males and females, regardless of the HPV vaccine product previously administered. The Advisory Committee on Immunization Practices (ACIP) recommendations for HPV9 includes the following:

  • Administration to male and female clients beginning at 11 to 12 years of age, but may begin as early as 9 years or age
  • Vaccination for the following clients who have not been vaccinated previously:
    • Females who are 13 to 26 years of age
    • Males who are 13 to 21 years of age
  • Males who are 22 to 26 years of age may also be vaccinated
