LTCOP Enhancements for HCS and TxHmL Forms to Be Implemented July 25, 2025

On July 25, 2025, the Texas Medicaid & Healthcare Partnership (TMHP) will implement enhancements to the Long-Term Care Online Portal (LTCOP). These updates will support changes to Home and Community-based Services (HCS) and Texas Home Living (TxHmL) program forms.

The LTCOP enhancements will identify new HCS and TxHmL form response errors, allowing users to correct data issues after submission. The forms will suspend in Provider Action Required status until the errors are resolved.

The new response codes and descriptions will be as follows:

  • PL-0005: The request cannot process because the Location type is ‘1’ or the Location ID is ‘OHFH’.
  • IP-0006: The request cannot process because the IPC Begin Date is earlier than the ID/RC Effective Date.
  • GN-0037: The request cannot process because the Service Authorization records are missing or have incomplete fields included in the transaction.
  • GN-0038: The request cannot process because the Address Update Flag is set, but no address was included in the transaction.
  • GN-0039: The request cannot process because the ID/RC is missing required fields in the transaction.
