Updated Substitute W-9 Form in PEMS

On October 30, 2025, the Texas Medicaid & Healthcare Partnership (TMHP) will replace the substitute W-9 form in the Provider Enrollment and Management System (PEMS) with the W-9 form on the Internal Revenue Service (IRS) website.

Providers can see what changes the IRS made to the W-9 form by going to the Instructions for the Requester of Form W-9 (03/2024) web page (irs.gov).

For help with completing the W-9 form, providers can visit the PEMS Instructional Site on tmhp.com. Select the appropriate page filters on the left side of the page and click Apply Filters. Scroll down on the Accounting/Billing Information page to find the W-9 information.

Note: If providers submit their application before October 30, 2025, but have a deficiency that they don’t resolve before TMHP updates the W-9 form, PEMS will display the new form, and the providers will be required to complete the new sections.
