Provider Enrollment on the Portal Instructions
Important: You must be enrolled in Traditional Medicaid before enrolling in CSHCN or Texas Health Steps. Select "Add to an Existing Enrollment," then click "Apply Filters" if you would like instructions for adding CSHCN or Texas Health Steps to your existing enrollment.

CSHCN Services Program Identification Form (Select)

Instructions for Completing the CSHCN Services Program Identification Form (Select) page.

  • Step 1: Please select a service
  • Choose the appropriate service that you are enrolling for.
  • The service you select must match the provider service that you selected on the Provider Type Identification form or what is currently on file for your Texas Medicaid TPI.
  • You can only select one provider type.
  • Click Save Selected Service(s).
  • Enter a TPI into the "Group TPI" field.
  • Select a TPI from the "Re-enrolling TPI" drop-down menu.
  • Select a specialty from the "Primary Specialty" drop-down menu.
  • The CSHCN Services Program specialty that you choose must match the Primary Specialty that you selected for your Traditional Medicaid enrollment.
  • If applicable, select a sub-specialty from the "Sub-Specialty" drop-down menu.
  • A sub-specialty is required for dentists.
  • Click Retrieve Taxonomies.
  • Select the appropriate taxonomy code in the "Available Codes" box.
  • Select your taxonomy based on the services that you provide. The available taxonomy codes are specific to the services rendered by the provider type and specialty that you have already selected.
  • Click Add Primary. The taxonomy code will move into the "Primary Taxonomy Code" box.
  • If applicable, click Add Secondary to move any applicable codes to the "Secondary Taxonomy Codes" box.
  • Click Continue and Save.
