Provider Enrollment on the Portal Instructions
Important: You must be enrolled in Traditional Medicaid before enrolling in CSHCN or Texas Health Steps. Select "Add to an Existing Enrollment," then click "Apply Filters" if you would like instructions for adding CSHCN or Texas Health Steps to your existing enrollment.

Instructions for Completing the Section C: Group Practice page.

Note: This page does not apply for Re-Enrollment or Revalidation applications.

Note: This page does not apply to Texas Health Steps Medical or Ordering and Referring Providers.

Note: This page does not apply to Facility, Performing, or Individual Providers.

  • Click Add a New Performing Provider.
  • Select Assign Enrollment to Performing Provider or Enroll Performing Provider.
  • If you are going to complete an application for the performing provider, select Enroll Performing Provider.
  • If you want the performing provider to complete an application, select Assign Enrollment to Performing Provider.
  • Note: The performing provider must have a TMHP portal account user name and password to complete the application. You must provide the performing provider’s username and NPI in order to assign the application.
  • Note: The Performing Provider must sign an agreement contract.
  • Click Add.
  • The performing provider application will open to the Provider Type Identification Form.
  • After the performing provider application is completed through the agreement page, click Continue and Save.
  • Note: You will be taken back to the Section C Group Practice page.
  • Click Continue and Save.
