LICENSE FOR USE OF CURRENT PROCEDURAL TERMINOLOGY, FOURTH EDITION ("CPT® ")

You, your employees and agents are authorized to use CPT only as contained in materials on the Texas Medicaid & Healthcare Partnership (TMHP) website solely for your own personal use in directly participating in healthcare programs administered by THHS. Reminder for All Providers Enrolled in Medicare: Submitting Paper Crossover Claims to Texas Medicaid

This is a reminder for all providers enrolled in Medicare who submit paper crossover claims to Texas Medicaid: You must submit the same information to Texas Medicaid that you received from Medicare.

Providers that submit the TMHP Standardized Medicare Advantage Plan (MAP) Remittance Advice Notice Templates to Texas Medicaid Healthcare Partnership (TMHP) must submit information in the Templates that is identical to the information on one of the following documents:

Remittance Advice (RA)
Remittance Notice (RN)
MAP Explanation of Benefits

Claims that do not contain identical information may be denied.
