HHSC Publishes Requirements for IPC Submissions for Individuals Requesting Services funded with General Revenue (IL 2021-32)

Last updated on

The Texas Health and Human Services Commission (HHSC) has published the requirements needed for an individual plan of care (IPC) submission request for individuals enrolling in the Community Living Assistance and Support Services (CLASS), Deaf Blind with Multiple Disabilities (DBMD), Home and Community-based Services (HCS) Program or when an individual’s IPC is being revised or renewed.

This information letter (IL) describes the documentation that a CLASS Case Manager, DBMD Program Provider, HCS Service Coordinator or HCS Program Provider must submit to HHSC to request the use of general revenue to pay for services above the individual cost limit of a waiver program.

More information can be found in the IL 2021-32 IPC Submissions for Individuals Requesting Services funded with General Revenue (PDF).
