Rural Hospital COVID-19 in Healthcare Relief Grant (RH-CHRG) Program, Tier 1: Direct Grant Awards, Action Required

The Texas Health and Human Services Commission (HHSC) disseminated a pre-award survey through SurveyMonkey to each rural hospital identified as eligible (PDF).

Each eligible rural hospital can receive $250,000 under the RH-CHRG program, Tier 1: Direct Grant Awards. To receive the direct grant award, eligible rural hospitals must first complete this pre-award survey to provide HHSC with information that will be used to populate the grant agreement; this includes information about the hospital’s legally authorized signatory for the purposes of the grant award. The survey is due to HHSC by January 12, 2022, at 5 p.m. Central Standard Time.

The survey was distributed to the on-file CEO email addresses for the hospitals listed in the PDF document linked above for eligible hospitals. If a hospital is unable to locate the pre-award survey email, please first check the email account’s junk folder. If you still cannot locate the pre-award survey email, contact the HHSC Provider Finance Department.
