You, your employees and agents are authorized to use CPT only as contained in materials on the Texas Medicaid & Healthcare Partnership (TMHP) website solely for your own personal use in directly participating in healthcare programs administered by THHS. Updated Instructions and Resubmission Guidelines for UB-04 CMS 1450 Paper Claims

On July 1, 2022, the instructions for billing provider taxonomy code placement in the UB-04 CMS 1450 paper claim form will be updated: the billing provider taxonomy code will be moved to block 81.

UB-04 CMS 1450 paper claim forms that are submitted with the billing provider taxonomy code in block 57 instead of block 81 will be accepted for 90 days. Forms that are submitted with the billing provider taxonomy code in the incorrect block after September 30, 2022, will be denied or returned to the provider.

The Texas Medicaid Provider Procedures Manual, Vol. 1, “Claims Filing,” and the Children with Special Health Care Needs Services Program Provider Manual, Chapter 5: “Claims Filing, Third-Party Resources, and Reimbursement,” will be updated to reflect this change.
