Reminder About Dual Entry in the CARE and TMHP Systems

Due to the data migration discussed in Information Letter No. 2022-18

 (IL 2022-18), Home and Community-based Services (HCS) and Texas Home Living (TxHmL) providers, local intellectual and development disability authorities (LIDDAs), and financial management services agencies (FMSAs) billing on behalf of Consumer Directed Services (CDS) are required to submit the following forms into both the Client Assignment and Registration (CARE) and Texas Medicaid & Healthcare Partnership (TMHP) systems if the forms have an effective or revision date prior to May 1, 2022, and they were not entered into the CARE system before close of business on April 6, 2022:

  • 8578 Intellectual Disability/Related Condition Assessments
  • 8582 TxHmL Individual Plans of Care
  • 3608 HCS Individual Plans of Care

Providers, LIDDAs, and FMSAs can use the effective date for the form to determine if it needs to be submitted in both CARE and TMHP systems. Failure to submit the form in both systems can delay or prevent payment.
