Now Accepting Nominations for Nursing Facilities to Participate in the GARDEN Civil Monetary Penalty (CMP) Project (FOR HOUSTON REGION ONLY)

The Texas Health and Human Services Quality Monitoring Program (QMP) plans to apply for Civil Monetary Penalty Reinvestment Funds (CMP) to implement the Eldergrow Garden project in 25 nursing facilities (NF) in the Houston area. The proposed CMP project would fund indoor mobile garden beds, plants, and supplies, as well as twice monthly therapeutic horticulture classes with an Eldergrow Educator.

Now is your chance to nominate your facility by completing this survey. Eligible facilities are limited to those in the following counties: Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery, and Waller. The survey explains recent changes to the CMP application process and a project description.
