AMA/ADA End User License Agreement
LICENSE FOR USE OF CURRENT PROCEDURAL TERMINOLOGY, FOURTH EDITION ("CPT® ")

You, your employees and agents are authorized to use CPT only as contained in materials on the Texas Medicaid & Healthcare Partnership (TMHP) website solely for your own personal use in directly participating in healthcare programs administered by THHS. You acknowledge that AMA holds all copyright, trademark and other rights in CPT.

Certain Provider Types to Complete CoCM Attestation

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to Medicaid members who are enrolled in their MCO. Administrative procedures, such as prior authorization, precertification, referrals, and claims and encounter data filing, may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the member’s specific MCO for details.

Effective February 1, 2024, the Attestation Form for Collaborative Care Model (CoCM) in Texas Medicaid will be available in the Provider Enrollment and Management System (PEMS). This form must be included in the provider’s enrollment record.

Note: Any providers that filled out the attestation form previously must complete the new attestation in their PEMS enrollment record.

Completing the Attestation Form in PEMS

Prior to delivering CoCM services, the primary care physician must attest in PEMS that their practice location has an established CoCM program. They must also attest that they are actively providing CoCM services consistent with the core principles and specific functional requirements of the model.

The following provider types are subject to CoCM attestations:

Note: The attestation form will not be available in PEMS before February 1, 2024.

A PEMS enrollment request that is submitted before February 1, 2024, may be subject to an enrollment deficiency review. Provider Enrollment may send the application back to select either Yes or No to the CoCM attestation question within the new CoCM Services section. This section is found on the Program and Service Participation Details page in PEMS.

The provider must answer the attestation question for each enrolled Acute Care practice location. The question is not required for disenrolled or disenrolling locations.

Providers that do not provide CoCM services should select No on the attestation question and should select No for each location at which they do not provide CoCM services.

In addition to answering the attestation question, providers must submit the CoCM Attestation Form on the License/Certification/Accreditations (L/C/A) page in PEMS and ensure that the attestation form is associated to each practice location at which they perform CoCM services.

Once a CoCM Attestation Form is on file, a provider may terminate their attestation for each location by:

Submitting an Existing Enrollment or Maintenance License Request.
Answering No to the question.
Disassociating the attestation form from each location. If a provider wants to terminate their attestation from all locations, they can terminate the form from the L/C/A page.
