Updates to Navigating Paper Claim Forms CBT Now Available

The updated Navigating Paper Claim Forms computer-based training (CBT) is available on the Texas Medicaid & Healthcare Partnership (TMHP) Learning Management System (LMS).

This interactive course offers detailed information that will help providers to complete the following forms:

  • The Centers for Medicare & Medicaid Services (CMS) 1500 Health Insurance Claim Form and UB-04 Uniform Bill (also known as CMS-1450)
  • The Texas Health and Human Services Commission (HHSC) 2017 Family Planning Claim Form
  • The American Dental Association (ADA) Claim Form (2024 version)

In this CBT, providers will learn about specific fields and how to review all the fields on a form, who should complete these forms, and the requirements for submitting paper claim forms.

Providers must have a username and password to access training in the LMS. To get a username and password, providers must create an account by completing the New User Registration form on the LMS homepage.
