Updated Drug Shortage Notification and Expedited PDL Form Available

Prescribers and stakeholders can use the revised Drug Shortage Notification and Expedited Formulary or Preferred Drug List Request (HHS Form 1315) to submit notifications and requests to the Vendor Drug Program (VDP). This includes the following:

  • Potential drug shortages that impact prescribing choice and pharmacy claims processing
  • Requests for the addition of drugs to the VDP formulary
  • Requests for changes to the preferred drug list (PDL)

VDP will respond to the requestor within 24 hours on reported drug shortages.

Note: Updates to this form comply with Texas Government Code, Section 531.072(g) as added by H.B. 3286, 88th Texas Legislature.
