TULIP Scheduled Maintenance Starting November 14

Licensees and providers who need to access TULIP should do so before Friday, November 14, 2025, as the system will be temporarily unavailable for scheduled updates.

TULIP is expected to be down from Friday, November 14, at 7 p.m. through Wednesday, November 19, at 5 p.m. Please note that this timeframe is an estimate and may change.

HHSC encourages all users to review their status and verify due dates in TULIP. If any renewals or fees are due during the scheduled system downtime, please complete those actions in advance. To ensure compliance with due dates, timely fee payment, and uninterrupted processing, all time-sensitive TULIP actions should be completed before November 14.

IMPORTANT: If you need to access TULIP and are able to do so before November 14, please log in and complete any necessary actions. If you do not need to access TULIP or update your information, no action is required. To self-report an incident while TULIP is offline, use the Complaint and Incident Intake phone line: 800-458-9858 (Monday through Friday, 7 a.m.–7 p.m.) or email. Email addresses vary by program; see the “Report Incidents” section below for program-specific email addresses. 

What You Need to Know

TULIP, the Texas Unified Licensure Information Portal, is undergoing significant updates to improve system performance and reliability. This outage is the first step toward HHSC's long-term goal of moving TULIP to a new platform that will be more user-friendly and intuitive. These updates are necessary to support system enhancements and will improve the system’s overall performance and reliability. Additional improvements to usability will be occurring over the next few years.

The system is used for submitting long-term care licensure applications, health care regulation Architectural Review requests, and reporting incidents.

TULIP will be unavailable from Friday, November 14, 2025, at 7 p.m. until Wednesday, November 19, 2025, at 5 p.m.

What You Need to Do

Report Incidents

While TULIP is offline, HHSC Complaint and Incident Intake (CII) reminds Long-Term Care providers that an initial self-reported incident must be submitted by filling out the CII Self-Report Email Template (PDF) and emailing it to ciicomplaints@hhs.texas.gov, or by calling and speaking to an agent at 800-458-9858 (Monday through Friday, 7 a.m.–7 p.m.).

For initial self-reported incidents submitted by phone or via email, providers can email the report (Form 3613-A /Form 3613) to ciiprovider@hhs.texas.gov or fax to 877-438-5827.

Health Care Regulation providers must fill out their corresponding form and email it to CII.HCQ@hhs.texas.gov or call and speak to an agent at 800-458-9858 (Monday through Friday, 7 a.m. – 7 p.m.).

Log In

After the TULIP updates are completed, all TULIP users will need to log in and reset their password. HHSC will provide instructions describing when and how to complete the password reset. 

If you do need to update your information in TULIP now, see below:

  • Visit the TULIP page before November 14 to update your information.
  • If you have a renewal date that falls within or near the system downtime, HHSC recommends completing any time-sensitive actions before November 14 to ensure due dates are met, fees are paid on time and processing delays are avoided.
