Activities Director Disclosure Requirement for DAHS Provider Enrollment

Day Activity and Health Services (DAHS) providers must disclose an activities director when they are applying for Texas Medicaid enrollment through the Provider Enrollment and Management System (PEMS). An activities director is considered a managing employee or principal and therefore must be listed in the “Owner/Creditors/Principals” section of the enrollment application.

To meet this requirement and avoid processing delays, DAHS providers should ensure that the following steps are completed in the enrollment application:

  • Enter an activities director for the enrolling entity.
  • Confirm that the activities director is listed within the “Owner/Creditors/Principals” section.
  • Confirm that the role in the Provider Organization drop-down is set to Director.
  • Confirm that the activities director has been disclosed in the Title or Duties field.
