TULIP Scheduled Maintenance

The Texas Unified Licensure Information Portal, TULIP, will be unavailable while HHSC is completing scheduled system updates. After the system update, all TULIP users must reset their passwords within 24 hours of receiving notification.

TULIP is scheduled to be down from Friday, Dec. 12 at 7 p.m. until Wednesday, Dec. 17 at 5 p.m. or as soon as possible. Note: Any updates or timeline changes will be communicated promptly on the TULIP website and through GovDelivery messages.

IMPORTANT: To avoid delays or missed deadlines, users should review upcoming renewal and application due dates now and complete your actions before the scheduled downtime. Taking care of these tasks early will help ensure your operations continue smoothly during the outage.

Public-facing search pages will remain active: The Nurse Aide/Medication Aide/ Nursing Facility Administrator Public Registry page, the Employability Status Check page, the Employee Misconduct Registry page, and the LTC Provider Search will remain active while TULIP is down for the migration. 

After the system update, all TULIP users must reset their passwords. HHSC will share password reset instructions once TULIP is back. The password reset email will contain a link. That link is only valid for 24 hours. You will also need to save the new site in your bookmarks and delete the old bookmark.

This outage is the first step toward HHSC's long-term goal of moving TULIP to a new platform that will be more user-friendly and intuitive. These updates are necessary to support system enhancements and will improve the system’s overall performance and reliability. Additional improvements to usability will be occurring over the next few years.

Reporting Incidents

Long-Term Care Providers:

If you need to self-report an incident while TULIP is offline, Long-Term Care providers must fill out the CII Self-Report Email Template (PDF) and email it to ciicomplaints@hhs.texas.gov, or call and speak to a Complaint and Incident Intake agent at 800-458-9858 (Monday through Friday, 7 a.m.–7 p.m.).

For initial self-reported incidents submitted by phone or via email, providers can email the report (Form 3613-A/Form 3613) to ciiprovider@hhs.texas.gov or fax to 877-438-5827.

HHSC will upload all information that is received while the system is down.

Health Care Regulation Providers:

If you need to self-report an incident while TULIP is offline, Health Care Regulation providers must fill out their corresponding form and email it to CII.HCQ@hhs.texas.gov or call and speak to an agent at 800-458-9858 (Monday through Friday, 7 a.m.–7 p.m.).
