Texas Medicaid Guidance in Response to Lyons Magnus Limited Voluntary Recall of Kate Farms Pediatric Standard 1.2 Vanilla Formula

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to eligible clients. Administrative procedures such as prior authorization, precertification, referrals, and claims/encounter data filing may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the client's specific MCO for details.

On July 28, 2022, the Lyons Magnus manufacturing plant announced a voluntary recall of five lots of the Kate Farms Pediatric Standard 1.2 Vanilla product.

The lot numbers issued in the recall are:

  • 2512
  • 3512
  • 4512
  • 5512
  • 6512

Formula with any of these lot numbers should be destroyed or returned to Kate Farms. Formula that does not have one of these lot numbers can continue to be used as normal.

Refer to the recall notification on the Texas Health and Human Services (HHSC) Special Supplemental Nutrition Program for Women, Infants and Children (WIC) web page, which contains information to determine whether a Kate Farms formula product is part of the recall.

Physicians and other health care providers can refer to the Kate Farms website for more information. Providers would have used the Healthcare Common Procedure Coding System (HCPCS) code B4160 to request any products that are now under the voluntary recall.
