Update to Texas Medicaid Guidance in Response to Lyons Magnus Voluntary Recall of Kate Farms Products

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to eligible clients. Administrative procedures such as prior authorization, precertification, referrals, and claims/encounter data filing may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the client's specific MCO for details.

This is an update to an article titled “Texas Medicaid Guidance in Response to Lyons Magnus Limited Voluntary Recall of Kate Farms Pediatric Standard 1.2 Vanilla Formula,” which was posted on the TMHP website on August 4, 2022.

The voluntary recall issued by Lyons Magnus manufacturing plant has been expanded. The updated recall issued on August 10, 2022, now includes additional Kate Farms products.

Lyons Magnus is conducting this voluntary recall due to the potential for microbial contamination, including the organisms Cronobacter sakazakii and Clostridium botulinum. This expanded recall impacts other products manufactured by Lyons Magnus. Click here for more information from the U.S. Food & Drug Administration (FDA).

The complete list of lot numbers for Kate Farms formulas listed below and how to identify whether a product is impacted by the recall can be found here.

  • Kate Farms Pediatric Peptide 1.5 Ready to Use (RTU) 8.45 oz.
  • Kate Farms Pediatric Standard 1.2 RTU 8.45 oz.
  • Kate Farms Peptide 1.5 RTU 11 oz.
  • Kate Farms Standard 1.0 RTU 11 oz.
  • Kate Farms Pediatric Peptide 1.0 RTU 8.45 oz.

The list of lot numbers for PediaSure Harvest and Ensure Harvest can be found here.

  • Pediasure Harvest RTU 8 oz.
  • Ensure Harvest RTU 8 oz.

Formula with any of these lot numbers should be destroyed or returned to Kate Farms. Formula that does not have one of these lot numbers can continue to be used as normal.
