Coming Soon: LTCOP HCS and TxHmL Transfer Form Submission Sequencing Enhancements

On December 18, 2025, the Texas Medicaid & Healthcare Partnership (TMHP) will update the Long-Term Care Online Portal (LTCOP) to improve form sequencing requirements and transfer processing for the Home and Community-based Services (HCS) and Texas Home Living (TxHmL) programs. These updates will help HCS and TxHmL waiver program providers and local intellectual and developmental disability authorities (LIDDAs) to submit forms accurately and reduce manual processing needs.

Transfer Form Process Improvements

TMHP will update LTCOP Individual Plan of Care (IPC) Transfer form submissions to automatically include the Individual Movement (IMT)-Individual Update form.

This change will simplify transfer processing because separate IMT-Individual Update submissions will not be needed after the IPC Transfer form has been processed.

Note: There will be no change to the current process for LIDDA reassignments or when the IMT-Individual Update is used without an IPC Transfer form.

County Filtering Refinements and Auto-Population of Individual’s Address

TMHP will update the LTCOP county filtering field to display only the counties that are served by the submitting LIDDA on the IPC Transfer form. This will also auto-populate the individual’s address based on the active location code that is entered on the IPC Transfer form. If the location code is inactive, the system will display an instructional error message. These changes will reduce administrative burden and data entry errors.

Note: LIDDAs will be able to update the individual’s own home/family home (OHFH) address on the IPC Transfer form.

TMHP will update user training materials to reflect these changes.

Future Related Enhancements

TMHP will implement a separate enhancement to combine Revision IPC and IMT-Individual Update form submissions. TMHP will provide more information about this update in a future article.
