January 2026 NDC-to-HCPCS Crosswalk Now Available for CAD Processing

The Texas National Drug Codes (NDC) to Healthcare Common Procedure Coding System (HCPCS) Crosswalk is now available for clinician-administered drug (CAD) processing.

Prescribers can use the CAD Search or download the crosswalk to review this quarter's changes. Search instructions are available. To download, choose the Excel or text file of the clinician-administered drug index in the “Data Files” section.

The NDC-to-HCPCS Crosswalk identifies relationships between NDC and Medicaid-reimbursable HCPCS codes. The Texas Health and Human Services Commission (HHSC) publishes the crosswalk using quarterly updates to the Centers for Medicare & Medicaid Services (CMS) rebate-eligible drug list and monthly updates to the First Databank pricing compendia.

HHSC wants prescribing providers to use the crosswalk to ensure that claims are accurately submitted.
