August 2026 TMPPM Update for Clinician Administered Drug Vitamin B12 (Cyanocobalamin) Injection

On August 1, 2026, the Texas Medicaid & Healthcare Partnership (TMHP) will update section 6.142, “Vitamin B12 (Cyanocobalamin) Injections,” in the Texas Medicaid Provider Procedures Manual (TMPPM), Outpatient Drug Services Handbook to remove diagnosis restrictions and indications for procedure code J3420.

Vitamin B12 injections (procedure code J3420) will continue to be a benefit of Texas Medicaid and should only be considered for clients with conditions that are refractory to, or have contraindication to, oral therapy.

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to Medicaid members who are enrolled in their MCO. Administrative procedures, such as prior authorization, precertification, referrals, and claims and encounter data filing, may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the member’s specific MCO for details.
