Reminder of Valid Electronic Signature Criteria

The Texas Medicaid & Healthcare Partnership (TMHP) would like to remind providers to meet the signature and date requirements for prior authorization and other required forms that are kept in the client record. Other required forms include plans of care, physician orders, evaluations, and letters of medical necessity.

The following are acceptable types of signatures:

  • Electronic or digital signatures that are created by software, which produces a digital system-generated date and time stamp or includes the logo or seal of the digital software that is used
  • An electronic signature audit trail

TMHP will not accept the following types of signatures:

  • A photocopy or ink stamp of a handwritten (wet) signature
  • Typed signatures
  • Stamped images of handwritten (wet) signatures

Note: Texas Medicaid managed care organizations (MCOs) must provide all medically necessary, Medicaid-covered services to Medicaid members who are enrolled in their MCO. Administrative procedures, such as prior authorization, precertification, referrals, and claims and encounter data filing, may differ from traditional Medicaid (fee-for-service) and from MCO to MCO. Providers should contact the member’s specific MCO for details.
