HHSC Announces Revised Form 2032, Escheatment of Consumer Funds

HHSC revised Form 2032, Escheatment of Consumer Funds, effective June 2026, including updates to the HHSC Accounts Receivable mailing address.

Assisted living facilities, intermediate care facilities for people with intellectual or developmental disabilities, and nursing facilities must use updated Form 2032 effective immediately.

Providers should discard older versions of the form and only use the revised form. The version date is listed on the top right side of the form. Using older versions of Form 2032 can cause significant delays.
