New PASRR Plan of Care Form Available September 1, 2026

The new Preadmission Screening and Resident Review (PASRR) Plan of Care form will be available in the Texas Medicaid & Healthcare Partnership (TMHP) Long-Term Care Online Portal (LTCOP) on September 1, 2026.

Local intellectual and developmental disability authorities (LIDDAs) will use the new form to create a plan of care for individuals who:

  • Have an intellectual or developmental disability (IDD).
  • Are currently admitted to a Medicaid-certified nursing facility.
  • Are eligible for Medicaid and PASRR.
  • The form will also automate requests to authorize habilitation coordination service units.

Note: LIDDAs will continue to submit claims for habilitation coordination services through the TMHP claims management system.

Using the PASRR Plan of Care Form

To create a PASRR Plan of Care, LIDDAs will select Initiate Plan of Care from the top of the current PASRR Comprehensive Service Plan (PCSP) form.

LIDDAs will be directed to the Initiate Form page, where all fields will be autofilled and read-only. The Type of Form field will be autofilled with “1013: PASRR Plan of Care (POC)” and the POC Type field will be autofilled with one of the following values based on the associated PCSP:

  • “1. Initial”
  • “2. Annual/Renewal”

To create a Revision Plan of Care, LIDDAs must initiate the revision from an existing Initial or Annual/Renewal PASRR POC form.
