Consumer Directed Services Publication Updates

The policy that prohibited Community First Choice (CFC) personal attendant services/habilitation (PAS/HAB) providers from living with the person receiving services in the Home and Community-based Services (HCS) and Texas Home Living (TxHmL) programs was removed from the following consumer directed services (CDS) forms:

The updated forms can be accessed from the CDS Forms link on the Consumer Directed Services Handbook page and are available in both English and Spanish where indicated. Updates to some of the Spanish versions are in progress.

The Instructions page for some of the forms was updated to reflect person-centered and plain language rules were needed. The Instructions page for Form 1735, the Employer and Financial Management Services Agency Service Agreement, includes instructions for forms 1735-cfc, 1735-hcs and 1735-txhml, the service provision requirements addendums.

The Spanish version of Consumer Directed Services (CDS) Option FAQs was published to the HHS website located on the Consumer Directed Services (CDS) | Texas Health and Human Services and the About CDS | Texas Health and Human Services webpages.
