Participant Information

Instructions for completing the Participant Information page

Temporary ID – this will be pre-populated for you. No action is needed.

Select the Application Type from the left navigation menu.

Enter the Name of the person signing up or registering as an Individual Transportation Participant (ITP) :

First Name – Enter your first name (must match name listed on Driver’s License)

Middle Name – Enter your middle name, if you have one

Last Name - Enter your last name (must match name listed on Driver’s License)

Suffix – Enter your suffix, if you have one ,( such as a Jr, Sr, etc.)

Social Security Number - Enter your “Social Security Number”

Date of Birth - Enter your “Date of Birth”

Gender - Select your “Gender” from the available drop-down menu.

ID Type -Select “Driver’s License” from the available drop-down menu.

ID Number - Enter your Driver’s License Number.

State Issuer - Select your “ID State Issuer” from the available drop-down menu. If the state that is on your current Driver’s License is Texas, select Texas; otherwise, select the State that matches your Driver’s License.

Issue Date - Enter the “Issue Date” for your Driver’s License.

Expiration Date - Enter the “Expiration Date” for your Driver’s License.

Primary Email address -Enter your email address as the “Primary Email Address”.

Secondary Email Address - Enter a “Secondary Email Address”, if you have one. If not, leave this field blank.

Click Verify Email. If the email address has not been verified with TMHP before, you will receive an email with a link to confirm the email address you entered is correct. Click the link in the email to update the Email Status to Verified for each email address provided.

Home Address

This section will be pre-populated for you. If you require edits to the home address, you can change any of the info as needed.

Address Line 1 – This is your street address, including the house number and street where you live.

Address Line 2 – if you live on 2 cross streets, the second street address can be added in this line.

City – This is the City where you live.

State – This is the state where you live.

Zip Code – This is the Zip Code where you live.

Zip Code +4 – This is the additional 4 digits for your zip code.

Click Verify Address.

If the address verification shows an error, and the home address entered is accurate, select “Continue with address entered”.

Vehicle Insurance Policy

Click +Add Vehicle Insurance Policy

This will take you to a new screen or sub-page to enter your Vehicle Insurance Policy information.

Vehicle Insurance Policy Number – Enter the Insurance Policy number. This is the number listed on your Vehicle Insurance card issued by the Insurance company.

Initial Issue Date – Enter the start date of your current Vehicle Insurance Policy.

Expiration Date – Enter the end date of your current Vehicle Insurance Policy.

Note: If your Vehicle Insurance Policy expires any time during TMHP’s review of your registration application or if you change policies, you will be required to add the updated Vehicle Insurance Policy information, in addition to supplying a copy of the updated or new Policy.

Click Save, this will take you back to continue completing the Participant Information page.

Important: The name listed on your vehicle insurance card, driver’s license, and Social Security card must be the same.

Maiden Name, Other Alias, Nicknames

If you have a Maiden Name, Other Alias or Nickname that you have ever used:

Click +Add Maiden Name, Other Alias, Nicknames. This will take you to a new screen or sub-page to enter your information.

Select the “Name” Type using the drop-down menu.

Enter the “First Name”

Enter the “Middle Name” (if you had one for the name being added)

Enter the “Last Name”

Enter the “Suffix”. (if you had one for the name being added)

Click Save. This will take you back to continue completing the Participant Information page.

Registrant Information

This section will show your current ITP registration status. Please note, this information will be blank and un-editable for New Registrations.

Registrant Status

Pending – Your New Registration Application has been received but has not been Approved yet.

Registration Periods

This section show the Registration Periods for your ITP Registration Record. Please note, this information will be blank and un-editable for New Registrations.

Begin Date: The date that the registration record was approved.

End Date: The date that the registration record was ended.

Click Save.

Access the left navigation and select the next available page to continue entering your registration application.

Instructions for reviewing the Participant Information page

Review the information below to ensure it is still accurate. If any changes are needed, provide updates to the field accordingly.

Temporary ID – this will be pre-populated for you. No action is needed.

Review/Update the Name of the person signing up or registering as an Individual Transportation Participant (ITP) :

Click Save, this will take you back to continue completing the Participant Information page.

Click Save. This will take you back to continue completing the Participant Information page.

Registrant Information

This section will show your current ITP registration status. Please note, this information cannot be edited by you or Provider Enrollment and is in a View-Only status.

Registration Status:

Enrolled – Your Registration Application is currently active.

Registration Periods

This section will show the Registration Periods for your ITP Registration Record. Please note, this information cannot be edited by you or Provider Enrollment and is in a View-Only status.

Begin Date: The date that the registration record was approved.

End Date: The date that the registration record was ended.

Click Save

Access the left navigation and select the next available page to continue entering your registration application.

Instructions for updating your email address.

Note: you will only be able to update your email address below. All other fields will be greyed out and not be open for any edits.

Primary Email address -Enter your email address as the “Primary Email Address”.

Secondary Email Address - Enter a “Secondary Email Address”, if you have one. If not, leave this field blank.

Click Verify Email. If the email address has not been verified with TMHP before, you will receive an email with a link to confirm the email address you entered is correct. Click the link in the email to update the Email Status to Verified for each email address provided.

Click Save

Once you are ready to submit, click Submit All Changes.

Instructions for updating your Insurance or Driver’s License information.

Note: you will only be able to update your Insurance and Driver’s License information below. All other fields will be greyed out and not be open for any edits.

Driver’s License Information

ID Type -Select “Driver’s License” from the available drop-down menu.

ID Number - Enter your Driver’s License Number.

State Issuer - Select your “ID State Issuer” from the available drop-down menu. If the state that is on your current Driver’s License is Texas, select Texas; otherwise, select the State that matches your Driver’s License.

Issue Date - Enter the “Issue Date” for your Driver’s License.

Expiration Date - Enter the “Expiration Date” for your Driver’s License.

Vehicle Insurance Policy

Click +Add Vehicle Insurance Policy.

This will take you to a new screen or sub-page to enter your Vehicle Insurance Policy information.

Vehicle Insurance Policy Number – Enter the Insurance Policy number. This is the number listed.

Initial Issue Date – Enter the start date of your current Vehicle Insurance Policy.

Expiration Date – Enter the end date of your current Vehicle Insurance Policy.

Note: If your Vehicle Insurance Policy expires any time during TMHP’s review of your registration application or if you change policies, you will be required to add the updated Vehicle Insurance Policy information, in addition to supplying a copy of the updated or new Policy.

Click Save, this will take you back to continue completing the Participant Information page.

Important: The name listed on your vehicle insurance card, driver’s license, and Social Security card must be the same.

Attachment

You will need to submit a copy of the new Insurance Policy or Driver’s License with this request.

Select “Click here to select files” to upload your documentation.

Select the document from your computer in the pop-up screen and click Open.

Repeat for all required documentation.

Click Save

Once you are ready to submit, click Submit All Changes.
