Are you ready to enroll?

Are you ready to edit your enrollment?

Select the Application Type from the left navigation menu.

Log in and select your request type.

Log in to the TMHP Provider Enrollment and Management System (PEMS) on tmhp.com.

Log in to the TMHP Provider Enrollment and Management System (PEMS) on tmhp.com.

Click Start New Enrollment from the Provider Management Module Tab of the PEMS Dashboard.

Verify NPPES Information

Verify that the information on the screen matches the NPI of the provider you are attempting to enroll.

Are you seeking enrollment due to a change of ownership (CHOW) and maintaining this NPI? Select Yes or No

If your entity has gone through a Change of Ownership, select Yes.  Continue to the next step.

If your entity has not gone through a change of ownership, click No. Proceed to “If you do not have an NPI, follow this step.”

Enter the Effective Date of the Change of ownership.

Select the change in ownership reason:

Change in Lease

Merger

Termination

To continue, you must check the following box:

When the NPI has been confirmed, click Begin Enrollment.

If you do not have an NPI, follow this step:

Select No if you are not eligible for an National Provider Identifier (NPI). Note that this is only if the National Plan and Provider Enumeration System (NPPES) will not issue an NPI because you are not a health-care professional.

Specify the type you are wishing to register or enroll as by selecting one of the following options:

Individual Transportation Participant—Individuals who register as participants for the Medical Transportation Program to drive clients to their medical appointments.

 Access the appropriate NPI/API segment associated to your TMHP User Account.

Select Provider Management and search the appropriate NPI/API associated to your TMHP User Account.

Atypical Provider –Entities or Individuals who are unable to obtain an NPI and are approved to enroll as an Atypical provider will be issued an Atypical Provider Identifier (API) in place of the NPI.  Only certain provider types are able to utilize this enrollment:

Consumer Directed Services Agency (CDSA) – Providers that are currently contracted with DADS to administer consumer-directed services (CDS).  Also known as Financial Management Services Agency. (FMSA)

Milk Donor Bank – Providers who enroll must adhere to quality guidelines consistent with the Human Milk Bank Association of Northern America.

Personal Care Services (PCS) - Providers that want to participate in the delivery of PCS must have one of the following HHSC licensures:

Personal Assistance Services (PAS) 

Licensed Home Health Services (LHHS) 

Licensed and Certified Home Health Services (LCHHS) 

Note: Licensed Home and Community Support Services Agencies (HCSSA) that are not Medicare certified may provide ONLY Personal Care Services (PCS) under Texas Medicaid CCP.

Service Responsibility Option (SRO) - To enroll in the Texas Title XIX Medicaid Program, Service Responsibility Option providers must complete the Texas Medicaid enrollment application. Providers of personal assistance services must submit their contract with HHSC as a Service Responsibility Option provider.

Are you enrolling as an organization or Individual? – Select one of the following options for your Enrollment Type:

Organization – If you are a Facility or Group, select this option.

Individual – If you are an Individual, Performing Provider, or Ordering/Referring provider, select this option.

Dependent on your "Enrollment Type", one of the following selections will be available.

Click the Ellipsis […] and then View to open the Enrollment Record.

Click Edit Enrollment Record.

Click Edit Enrollment Record button at the top right of the screen.

Locate Request Type: Reenrollment.

Locate Request Type: Revalidation.

Locate Request Type: Maintenance.

Locate Request Type: Existing Enrollment.

Existing Enrollment—Select this option if you have a new program(s) or practice location(s) to add to your enrollment record.

Social Security Number—for Individual Transportation Participants (ITPs) or sole proprietorships. 

Tax Identification Number – for all other enrollments.

To continue, you must check the following box:

Click Begin Enrollment/Registration.

Reenrollment—For Affordable Care Act (ACA) reenrollments, select this option if you have passed your revalidation window or if you are attempting to reactivate your existing enrollment record after disenrollment.

Revalidation—For Affordable Care Act (ACA) revalidations, select this option if you are within your revalidation window as listed on the revalidation notification.

Maintenance – Provider Information – Existing providers who provide an update to their first name, last name, or both

Maintenance – Licenses - Existing providers who provide updated active renewal license information

Maintenance – Disclosures - Existing providers who are editing information associated with their disclosure history

Maintenance – W9/Tax Information - Existing providers who have a change or correction required to the Tax Information

Maintenance – Ownership/Controlling Interest – Existing providers who have changes in "Ownership/Controlling Interest" information, including but not limited to adding or removing any owners/creditors/principals

Maintenance - Programs – Existing providers who are providing an update to their existing programs or adding additional programs to their provider record

Maintenance – Attachments - Existing providers who require a change of Texas Identification Number or a change in Direct Deposit information

 Maintenance – Provider Information-Change Email – Existing providers who require updates to primary or secondary email addresses

 Maintenance – Practice Location – Demographics – Existing providers who are editing demographics information to an existing practice location

Maintenance – Practice Location – Address change - Existing providers who are editing information associated to an existing practice location on their enrollment record.

Under Request Action, select Create Request.

Note: Each request type will indicate the available Request Action. If a specific request type is already in process, that request must be completed before a new request of the same type can be started.

If you are the Group admin entering the application on behalf of the performing provider, follow the steps below:

Log in to Provider Enrollment and Management System (PEMS) on tmhp.com.

 Access the appropriate Group NPI/API segment.

Click the Ellipsis […] and then View to open the Enrollment Record.

Navigate to the Practice Location Information page. Under Performing Providers, select +Add Performing Provider.

Enter the NPI for the enrolling performing provider and click Validate.

The performing provider’s information from NPPES will be shown within the modal.

Select a practice location that the performing provider provides services.

This is one of the locations currently on file as a Practice Location for the group.

Click Add Performing. This will create a new request.

If you are enrolling as the performing provider, follow the steps below:

Log in and Select your Request Type.

Log in to Provider Enrollment and Management System (PEMS) on tmhp.com.

Select Start New Enrollment from the Provider Management Module Tab of the PEMS Dashboard.

Are you seeking enrollment with a National Provider Identifier (NPI)? – Select Yes or No

Locate Request Type Revalidation.

Existing Enrollment—Select this option if you have a new program(s) or practice location(s) to add to your enrollment record.
